As a special projects coordinator, Jacob provides technical assistance to LPHAs. Jacob's projects often involve direct work with LPHAs on efforts to improve community health and soliciting partner feedback; this regularly includes community health assessments and community health improvement plans. In his free time, he can be found at the nearest disc golf course, playing chess or walking his dog, C.J.
